This form authorizes recurring automatic withdrawals for tuition and other applicable school charges. If you only want to submit a check electronically or authorize individual payment amounts, use the electronic-check form instead.

This auto-pay form only needs to be filled out once and submitted per family.  Please only refill under the following circumstances:

  • Your banking information has changed
  • Your prior submission (of this form specifically) did not go through correctly

I authorize The Crescent Academy to initiate recurring automatic bank drafts from the bank account I have provided for all applicable tuition, registration fees, school supply fees, and other school-related charges for each student enrolled under my account.

Unless otherwise stated in writing by The Crescent Academy, I authorize payments to be deducted according to the following schedule:

Monthly Tuition Payments
Monthly tuition payments will be automatically deducted on the 1st day of each month from September 1 through June 1.

Annual Registration Fee
The annual registration fee for the upcoming school year will be automatically deducted on July 1.

Annual School Supply Fee
The annual school supply fee for the upcoming school year will be automatically deducted on August 1.

If a scheduled payment date falls on a weekend, holiday, or other non-banking day, I understand that the payment may be processed on the next available banking day.

I understand that tuition, fees, financial aid, and account balances may change from year to year. I authorize The Crescent Academy to charge my bank account according to the current tuition schedule, fee schedule, and balance reflected in my MySchool account.

This authorization will remain in effect each year for all students enrolled under my account unless and until I revoke it in writing in accordance with The Crescent Academy’s policies. I understand that revocation of this authorization does not cancel any tuition, fee, or other balance owed to The Crescent Academy.

I understand that it is my responsibility to ensure that sufficient funds are available in my account on each scheduled payment date. If an automatic payment is declined, returned, or rejected due to insufficient funds, closed account, incorrect banking information, stop payment, or any other banking issue, a $30.00 returned payment/bank transaction fee will be added to my account, in addition to any outstanding balance and any fees charged by my own financial institution.

I also understand additional delinquent charges may apply per policy.

I further understand that The Crescent Academy may contact me for updated payment information or require an alternative payment method if an automatic payment is declined or returned.
